Program Schedules

Please use this checklist as a tool to assist you through the enrollment procedure. Do not hesitate to contact the Danada office at (630) 668-6012, Tuesday through Saturday, 9:30 a.m. to 6:00 p.m. if you have any questions.

• Please complete a separate application form for each program or class that you wish to register for.
• Indicate a maximum of three class choices for the desired program on your application.
• All checks are to be made payable to: Forest Preserve District of DuPage County.
• Credit card numbers must be clear and legible. Visa, Mastercard & Discover accepted .
• Read and understand the cancellation policies.
• Read and sign all waivers (2 separate forms). Parents or guardian must sign for applicants under 18 years of age. (Faxed waiver forms are not accepted, we must have an original signature.)

INCOMPLETE AND INACCURATE APPLICATIONS WILL NOT BE PROCESSED AND WILL BE RETURNED.
